And, at the bottom of the page you will find a link to download a word document that you can edit for your bookings.  Anyone is free to use, or not use, any parts of this form for your bookings*.  Also, even if you don’t use a booking agreement, I suggest that you consider using at least the top of the form to collect the information you need for your bookings.

By doing a word replace for “Your Company”, you can switch to your name or your company.
